Discharge Coordinator

The South Calgary Primary Care Network (SCPCN) discharge coordinator is responsible for coordinating and communicating patient information on admissions and discharges for the SCPCN population. The
key responsibilities of this role include:

  • Verification that patient admission information is correct (correct physician identified)
  • Communication of patient discharge information to appropriate SCPCN physician or program
  • Booking follow-up appointments for unattached SCPCN patients using the reserved appointment slots
  • Receiving and responding to routine requests for the SCPCN services.

The primary goal of this program is to improve coordination of care for patients, by facilitating the flow of information between acute care and community.
