SCPCN Family Care Clinic

The Family Care Clinic (FCC) was created  to provide comprehensive, coordinated primary care services to families living within the SCPCN boundaries, who currently do not have a family physician. The primary objective is to attach patients to a family physician.

Services Provided:

  • Treatment of acute and episodic illness
  • Screening & disease prevention
  • Follow-up & support for patients discharged from the hospital
  • Management of chronic conditions
  • Walk-in services

Eligibility:

  • Patients accepted for care at the SCPCN Family Care Clinic must:
    • Live within the boundaries of the SCPCN (south of Anderson Road to city limits)
    • Have no family physician
    • Have a valid Alberta Health Care number
    • Patients can be referred by other clinics or health care facilities or, self-refer by calling the clinic

Referral Process:

  • Patients may self refer by calling the clinic or be referred by outside health care agencies.

Hours of Operation:

  • Monday to Friday
  • 9:00 am to 4:30 pm
