SCPCN Family Care Clinic
The Family Care Clinic (FCC) was created to provide comprehensive, coordinated primary care services to families living within the SCPCN boundaries, who currently do not have a family physician. The primary objective is to attach patients to a family physician.
Services Provided:
- Treatment of acute and episodic illness
- Screening & disease prevention
- Follow-up & support for patients discharged from the hospital
- Management of chronic conditions
- Walk-in services
Eligibility:
- Patients accepted for care at the SCPCN Family Care Clinic must:
- Live within the boundaries of the SCPCN (south of Anderson Road to city limits)
- Have no family physician
- Have a valid Alberta Health Care number
- Patients can be referred by other clinics or health care facilities or, self-refer by calling the clinic
Referral Process:
- Patients may self refer by calling the clinic or be referred by outside health care agencies.
Hours of Operation:
- Monday to Friday
- 9:00 am to 4:30 pm
