Health Management Clinic

The Health Management Clinic (HMC) was created with the goal of helping patients manage their chronic health concerns. Whether you are looking for motivational support, education, assistance with setting goals, or having a program designed just for you, the Health Management Clinic provides access to team of health care professionals to help you live with and manage your chronic health concerns. 

Services Provided:

•  Review and manage chronic disease risk factors
• Support for follow-up and self management
• Education, counseling and navigation of available resources
• Nutrition and exercise counseling

Eligibility:

• You have a chronic health concern such as:
  •  Diabetes
  • High blood pressure
  • High cholesterol
  • Weight (obesity) concerns

Referral Process:

• Physician Referral Required: Yes
• Self Referral: No

Hours of Operation:

• Monday to Friday
  • 8:30 am to 4:30 pm
  • Evening appointments available
