Low-Risk Maternity ClinicThe Low Risk Maternity Clinic program was established by the South Calgary Primary Care Network in 2006. The goal of this program is to provide patients with optimal care throughout their pregnancy. Whether it is your first baby or your fifth, planned or unplanned, our team can help you obtain the best possible care throughout your pregnancy. This clinic provides the necessary care for patients who reside with the SCPCN boundaries and who either do not have a family physician or have a family physician that does not provide routine maternity care. Clinic Forms and Handouts:If you have booked your appointment at the clinic please complete the following forms. If possible please fax the completed forms back to the clinic (403-452-7170) prior to your first appointment. If you do not have access to a fax machine please bring them to your first visit.
