Health Management

The Health Management program is designed to help patients manage their chronic health concerns – diabetes, high blood pressure, high cholesterol, and weight (obesity) concerns. Our multidisciplinary team includes registered nurses, dietitians, and social workers who all offer motivational support, education, goal-setting, and a personalized program to meet your needs.

Services Provided

After your doctor refers you to the program, the team can

  • Help you learn to manage your chronic condition
  • Show you how to take your medicines
  • Provide information about reading nutrition labels and preparing healthy meals and snacks
  • Support you with losing weight and safely starting an exercise program
  • Assist you in setting realistic goals to help you make healthy lifestyle changes
  • Help you stay motivated as you make important lifestyle changes

At your first visit, we will spend time with you to learn more about your health concerns and the challenges you may face in your everyday life. We may ask you about your goals and priorities, what changes you want to make, what has worked for you in the past, your biggest challenges, and how you define success. Group classes and one-on-one appointments are available.

The multidisciplinary team will then develop a program that is specially designed for you, so you can set your own goals and measure success in your own way.

Referral Process

Ask your doctor if you could benefit from the Health Management program. If so, your doctor will refer you, and we will contact you directly to set up your first appointment. You are eligible for one year of services, at no cost to you.

For patients who are meeting with a registered dietitian, please fill out the 3-Day Food Journal below. You may either complete it by hand or complete the fillable form and print it to bring with you to your appointment.
